Perinatal Navigator - Job Overview

Schedule: Monday-Friday (8AM-4:30PM) pr (8:30AM-5PM) | Rotating Schedule |

  • Engage in outreach and service coordination to enhance maternal and infant health.

Key Responsibilities

  • Conduct point-of-care testing such as HIV and pregnancy tests, alongside health screenings and risk assessments.
  • Assist clients with scheduling and follow-up appointments to ensure continuity of care.
  • Reach out to and engage with pregnant individuals needing support.
  • Provide culturally sensitive education on prenatal care, nutrition, and healthy pregnancy practices.
  • Conduct brief screenings of patient needs, including behavioral, social, and medical aspects.
  • Assist patients in setting and achieving health-related goals.
  • Implement comprehensive retention strategies for OB and pediatric patients.
  • Guide patients in navigating healthcare systems to achieve their health objectives.
  • Connect clients with community resources like WIC, housing, transportation, and behavioral health services.
  • Document all patient services in the electronic health record system.
  • Collaborate as a member of an interdisciplinary clinic and care team.
  • Meet performance metrics, including those related to grant and contract requirements.
  • Participate in various meetings, trainings, and workshops, including travel to different Legacy clinics.

Minimum Qualifications

  • 2-3 years of relevant experience.
  • Experience in a healthcare setting.
  • Experience with computers and computer programs.
  • Ability to provide point-of-care testing within 3 months if/as required per site assignment.
  • Ability to demonstrate core competencies within 3 months of hire.
  • Bachelor’s Degree in a related field with a minimum of 1 year of relevant experience preferred.
  • Bilingual English/Spanish.
  • Experience working in an electronic health record system.
  • Experience working with OB and Pediatric patient populations.
  • Experience working with sexual minorities and gender non-binary persons.
  • Experience providing education about pregnancy and/or the postpartum period.
