Care Team Assistant - Job Overview

Schedule: Monday-Friday (8AM-5PM)

Key Responsibilities

  • Ignite communication with patients—answer calls with flair, and ensure they leave every interaction with a smile!
  • Set your sights on pre-visit planning and ensure patients have everything they need before the main event.
  • Navigate the cosmos of care coordination by managing smooth transitions and supercharging service access.
  • Assist with medication matters like a pro, ensuring refills and authorizations sail smoothly.
  • Link arms with referral coordination staff for tip-top documentation and process wins.
  • Champion respect, teamwork, and safety—because we all win when we play as a team!
  • Follow Legacy Community Health's guidebook to continuously soar in safety and operational brilliance.

Minimum Qualifications

  • High school education or equivalent; a Medical Assistant Certification makes you an ace in the healthcare deck.
  • Preferably 2+ years of rocking it in a lively ambulatory clinical setting.
  • Speaking Spanish? You’ve got the golden ticket, especially depending on where you strut your stuff!
  • Proficiency with electronic health record systems—be the EHRS wizard we need!
  • Communication and customer service superpowers to positively impact every patient encounter.
  • Able to thrive in a fast-paced, multitasking environment, armed with remarkable organizational skills.
  • Typing and computer skills to keep the data-driven engine running smoothly.
  • Guard confidential and sensitive information like a true secret-keeper.
