Prenatal Navigator - Job Overview

Schedule: Monday-Friday (8AM-4:30PM or 8:30AM-5PM) | Schedule Dependent on LSW's Walk-In Testing Schedule

Key Responsibilities

  • Implement innovative health education programming to enhance patient knowledge.
  • Conduct pregnancy testing, health screenings, and risk assessments using advanced methodologies.
  • Empower patients in setting health goals and navigating care systems effectively.
  • Document patient services accurately in electronic health records.
  • Manage dynamic patient schedules with expertise.
  • Collaborate on interdisciplinary care teams to provide comprehensive health solutions.
  • Maintain robust referral processes and up-to-date resource lists.
  • Develop and innovate educational materials for maximum impact.
  • Participate in the development of groundbreaking health education curricula.
  • Consistently meet performance metrics and engage in strategic meetings.
  • Provide cutting-edge training and sustain professional acumen.

Minimum Qualifications

  • High School Diploma or equivalent with 3 - 5 years of relevant experience.
  • Preferred: Bachelor's Degree in a related field with at least 1 year of experience.
  • Proven experience in direct education within diverse cultural environments.
  • Proficiency in healthcare, digital tools, and innovative patient engagement techniques.
  • Ability to conduct health screenings and demonstrate Prenatal Navigator competencies.
  • Bilingual English/Spanish is highly desirable.
  • Experience with electronic health records and patient care in specialized areas such as HIV, gender non-binary, or chronic conditions.
  • Resilience in managing physical and mental demands.
  • Agility in adapting to varied work environments and technological advancements.
