Key Responsibilities

  • Conduct point-of-care and digital health testing, including HIV and pregnancy testing, utilizing state-of-the-art technology.
  • Use modern health screenings and risk assessments to deliver personalized health solutions.
  • Implement innovative outreach strategies to engage and support pregnant individuals.
  • Provide culturally sensitive, data-driven education on prenatal care, nutrition, and healthy pregnancy practices.
  • Execute brief, tech-enabled screenings of behavioral, social, and medical needs, facilitating readiness for health behavior change and tracking progress.
  • Empower patients to set and achieve health goals through motivational interviewing and goal-setting techniques.
  • Develop and implement comprehensive, digital-first retention strategies for OB and pediatric patients.
  • Streamline patient navigation through integrated care systems, ensuring seamless achievement of health goals.
  • Leverage community resource connections (WIC, housing, transportation) to enhance patient support.
  • Document all patient interactions in the electronic health record, utilizing Legacy's advanced EMR system.
  • Collaborate with an interdisciplinary team using agile methodologies to foster effective stakeholder relationships.
  • Meet performance metrics, ensuring alignment with grant and contract requirements.
  • Participate in clinics, team meetings, workshops, and trainings to advance expertise in healthcare innovation.
  • Travel to different Legacy clinics to promote cohesive, cross-location healthcare strategies.
  • Embrace other duties as assigned, contributing to a culture of continuous improvement.

Minimum Qualifications

  • High School Diploma or equivalent, with a forward-thinking approach to education and training.
  • 2-3 years of relevant experience in a healthcare setting, with a focus on digital health.
  • Proficiency in computers and advanced health technology systems.
  • Ability to deliver point-of-care testing, aligned with site-specific requirements, within 3 months.
  • Demonstration of core competencies within 3 months of hire.
  • Bachelor's Degree in a related field preferred, with emphasis on digital health innovation.
  • Bilingual English/Spanish, enhancing communication and community engagement.
  • Experience in electronic health record systems, streamlining healthcare delivery.
  • Experience with OB and Pediatric patient populations, leveraging technology for improved outcomes.
  • Experience working with sexual minorities and gender non-binary persons, promoting inclusive healthcare practices.
  • Keen ability to provide education about pregnancy and postpartum, utilizing tech-based methodologies.
