Perinatal Care Coordinator - Job Overview

Schedule: Monday-Friday (8AM-4:30PM or 8:30AM-5PM)

Key Responsibilities

  • Innovate health education programming in both one-on-one and group settings.
  • Conduct sophisticated screenings and track patient progress using data-driven methodologies.
  • Provide point-of-care services including health screenings and risk assessments.
  • Empower patients to set and achieve transformative health goals.
  • Manage a caseload of Legacy OB patients, ensuring access to essential services.
  • Facilitate patient screenings and support personalized care plans.
  • Strengthen hospital partnerships for newborn and postpartum care coordination.
  • Document services and manage schedules using advanced electronic health records.
  • Serve as a conduit between clients, healthcare providers, and community agencies.
  • Lead outreach events, workshops, and community engagements.
  • Monitor client progress and contribute to program evaluation and reporting.
  • Identify service gaps and advocate for systemic healthcare improvements.
  • Maintain dynamic referral networks and educational materials.
  • Contribute to health education content and curricula development.
  • Engage in performance metrics, meetings, and inter-clinic activities.
  • Embrace other duties as assigned, focusing on innovative solutions.

Minimum Qualifications

  • High School Diploma or equivalent, with a focus on healthcare innovation.
  • 3 - 5 years of experience in cutting-edge healthcare environments.
  • Proven experience in direct education to diverse client populations.
  • Experience with OB and Pediatric patient populations in future-focused settings.
  • Strong experience in healthcare settings, leveraging innovative practices.
  • Proficiency with computers, computer programs, and electronic health records.
  • Capability to provide point-of-care testing with site-specific advancements.
  • Demonstrate core competencies within 3 months of hire.
  • Bilingual English/Spanish is desirable for enhanced communication.
  • Experience with sexual minorities and gender non-binary persons is desirable.
  • Desirable experience in providing education about pregnancy and/or postpartum.
