Care Team Assistant (Patient Services) - Job Overview

Schedule: Monday-Friday (7:30AM-4:30PM)

Key Responsibilities

  • Act as the central hub for communication between patients, the call center, the care team, and family/caregivers.
  • Facilitate patient interactions, manage lab results, and coordinate pre-appointment planning.
  • Optimize medication management processes, including refills and prior-authorizations.
  • Prepare essential documentation for provider review and signature.
  • Identify patients requiring additional services, ensuring a proactive approach to care.
  • Coordinate care transitions, including hospital discharge records and specialty note management.
  • Efficiently address urgent tasks with providers to ensure prompt action.
  • Maintain a respectful, courteous environment for patients, families, and team members.
  • Promote a culture of teamwork and effective collaboration.
  • Adhere to strict safety procedures and uphold HIPAA compliance.

Minimum Qualifications

  • High school diploma or equivalent, with a preference for Medical Assistant Certification or two years of clinical experience.
  • Mandatory BLS/CPR certification to ensure preparedness in critical situations.
  • Fluency in Spanish is highly advantageous, enhancing patient communication.
  • Proficiency in electronic health record systems, supporting a data-driven approach.
  • Exceptional communication and customer service skills to foster patient trust.
  • Strong multitasking capabilities in a dynamic, fast-paced setting.
  • Advanced typing and computer skills for seamless digital operations.
  • Proven ability to handle confidential and sensitive information with integrity.
