Behavioral Health Representative I (YES Prep) - Job Overview

Schedule: Monday-Thursday (7AM-4:15PM) and Friday (7AM-3PM)

Key Responsibilities

  • Optimize provider schedules to enhance patient access and engagement.
  • Manage electronic health records and continuity of care lists efficiently.
  • Arrange language interpretation services to ensure inclusive communication.
  • Facilitate new patient registration, streamlining intake processes for optimal efficiency.
  • Conduct vital signs screenings and risk assessments for new patients.
  • Coordinate and manage referrals to support comprehensive care pathways.
  • Provide real-time communication between providers and patients for seamless care delivery.
  • Oversee waitlists for Behavioral Health Services to maximize access and capacity.
  • Process and follow up on Behavioral Health internal service orders with precision.
  • Assist in triaging behavioral health emergencies for rapid response.
  • Perform phlebotomy, injections, EKGs, and drug screenings as required.
  • Handle data entry, general office duties, and provide administrative support at the clinical site.
  • Promote effective team collaboration and respect for all patients and staff.
  • Ensure adherence to safety protocols and participate in required training programs.

Minimum Qualifications

  • BLS (CPR & AED) Certified, with a forward-thinking approach to patient safety.
  • Three to six months of related experience, with a preference for candidates with a vision for digital healthcare innovation.
  • One-year certificate from college or technical school preferred, highlighting your commitment to continuous learning.
  • Familiarity with Behavioral Health practices and a passion for pioneering patient support.
  • Proficiency in Microsoft Word, Outlook, and Excel for efficient data management and communication.
  • Excellent communication skills for dynamic interaction with providers and patients.
  • Strong leadership ability for innovating program management and driving change.
  • Insight into community agencies and resources to enhance care coordination.
  • Capability to manage multiple activities in an agile environment.
  • Preferred bilingual proficiency in English and Spanish to support diverse community needs.
