Care Team Assistant (LSJ) - Job Overview

Schedule: Monday-Friday (8AM-5PM) and 1 Saturday Per Month

  • Be the vital contact point and informational nexus for patients, families, and caregivers.
  • Provide dynamic clinical and clerical support aligned with Patient Centered Medical Home (PCMH) standards.
  • Leverage digital tools to ensure quality, compassionate care delivery.
  • Collaborate within a tech-advanced team to drive forward-thinking healthcare solutions.
  • Contribute to the mission of improving community health outcomes through innovative practices.

Key Responsibilities

  • Utilize protocol-based triage systems to effectively manage patient communication and inquiries.
  • Ensure timely patient updates through calls or automated letters regarding critical health information.
  • Drive proactive pre-visit planning by reviewing schedules and ensuring data availability.
  • Facilitate seamless care coordination, supporting transitions and ancillary services access.
  • Optimize medication management processes through refills and authorization support.
  • Streamline administrative processes by initiating and managing forms for provider review.

Minimum Qualifications

  • High-school level education or equivalency required.
  • Certified Medical Assistant or equivalent with at least 2 years of clinical experience.
  • Proficiency in EHRS system and adept at secure information management.
  • Strong multitasking abilities in a fast-paced, tech-driven environment.
  • Exceptional communication and customer service skills.
  • Fluency in Spanish preferred (based on service location).
  • Certification in BLS/CPR mandatory.
  • Demonstrated organizational skills and proficient typing and computer skills.
