Care Team Assistant - LSC (Job Overview)

Schedule: Monday-Friday (8AM-5PM)

Key Responsibilities

• Serve as the frontline responder for patient communication, ensuring effective coordination with the care team.
• Facilitate comprehensive pre-visit preparations to enhance patient experience.
• Support seamless transitions of care, ensuring continuity and quality of service.
• Manage the Care Coordination Desktop, keeping patient information organized and accessible.
• Assist in medication management, ensuring patients have the resources they need.
• Initiate the completion of critical forms to support patient care plans.
• Collaborate on referrals, connecting patients with necessary community resources.

Minimum Qualifications

• High School Diploma or equivalent.
• Medical Assistant Certification or equivalent experience preferred.
• Alternatively, 2 years of relevant experience in an inpatient or outpatient setting.
• 2 years of experience in an ambulatory clinical setting is preferred.
• Bilingual in English and Spanish is preferred, enhancing communication with a diverse patient population.
