About the Position

As a vital member of our team, the Ryan White Program Outreach Worker is instrumental in bridging the gap for clients teetering on the edge of care disengagement. This pivotal role is dedicated to reigniting client engagement with essential healthcare services, ensuring they harness the full spectrum of support available. Situated in the vibrant city of Houston, TX, this opportunity offers a platform to drive meaningful changes in the health landscape.

The Outreach Worker (OW) delivers proactive, field-based services, tailored to the unique needs of our clients. With a focus on medical, mental health, and psychosocial services, you will be the advocate clients rely on to eliminate barriers and forge pathways to health and wellbeing.

Key Responsibilities

  • Re-engage individuals diagnosed with HIV who are out-of-care, facilitating their return to HIV medical care and ART.
  • Interact with patients via telephone and in-person to advocate, identify, and meet their needs, ensuring seamless linkage to HIV care.
  • Customize work methodologies to address the diverse care needs, including physical, psychosocial, and cultural dimensions, of each patient served.
  • Cultivate trusting relationships with community partners, patients, and their families, providing robust support and encouragement.
  • Facilitate access to essential services such as transportation and language translation to dismantle care barriers.
  • Perform brief interventions for patients with low/intermittent needs, including renewals and service information.
  • Offer information, referrals, and support to streamline linkage to medical, psychosocial, and basic needs services.
  • Document all services provided in the Electronic Health Record System (EHRS) within 72 hours.
  • Advocate vigorously on behalf of clients to bridge service gaps and overcome service barriers.
  • Collaborate with Legacy staff and providers to assist with intricate system navigation, including Eligibility and Behavioral Health.
  • Work with community-based agencies to integrate social supports within the broader community.
  • Continuously evaluate service quality and efficacy through regular re-evaluation with peers and supervisors.
  • Engage actively in Legacy's Performance Improvement and Quality Assurance Program, upholding a dignified and respectful interaction with all patients, families, and team members.

Qualifications

  • High School Diploma or GED required.
  • Minimum of six months' experience working with or volunteering with Persons Living with HIV/AIDS (PLWH) is required.
  • Valid Driver License as driving duties are part of the role.
  • Preferred knowledge of Ryan White grants and services.
  • Prior experience in outreach efforts to engage individuals in HIV care is preferred.
  • Experience with the EPIC system is preferred.
  • Bilingual proficiency in Spanish is preferred.
