About the Position
The Service Linkage Worker provides services within a broad range of focus areas to assist patients in accessing and adhering to care. In addition, the SLW provides services to clients who do not require intensive case management services, and these include the provision of information, referrals and assistance with linkage to medical, mental health, substance abuse and psychosocial services as needed; advocating on behalf of clients to decrease service gaps and remove barriers to services helping clients to develop and utilize independent living skills and strategies. (This is a grant funded position)

Key Responsibilities

  • Assist patients in navigating service delivery systems and agency procedures.
  • Work with medical case managers and other clinic staff to ensure patient care plans are implemented.
  • Monitor patients' progress by reviewing attendance at HIV primary care appointments and by following up on status to ensure any referral appointments have been made and kept.
  • Assist patients to obtain or arrange for services such as transportation or language/translation services to eliminate possible barriers to care.
  • Conduct assessments to determine medical and psychosocial history and current service needs according to applicable standards of care or study requirements
  • Create care plans for referred patients/families to provide guidance and assistance with meeting care goals.
  • Provide information, referrals, and assistance with linkage to medical, psychosocial, and basic needs services
  • Adapts work methods to specifically accommodate the population ' specific care needs (physical, psychosocial, cultural, age /developmental) of each patient served.
  • Document service provided in EHRS client record within 72 hours
  • Assist with accessing specific programs and supports to address psycho-social needs both within Legacy and in the larger community.
  • Advocate for and on behalf of clients and families to decrease service gaps and remove barriers to services.
  • Connect with relevant Legacy staff and providers to assist with system navigation, including Eligibility, Patient Assistance, Behavioral Health, Health Navigation, etc.
  • Collaborate with community-based agencies to support linkage to social supports within the larger community

Requirements

  • Bachelor's Degree from an accredited college or university with a major in social or behavioral sciences.

  • Documented paid work experience in providing client services to Persons Living With HIV (PLWH) may be substituted for the bachelor’s degree requirement on a 1:1 basis (1 year of documented paid experience may be substituted for 1 year of college) --or-- High School Diploma or GED (And) Five (5) Years paid work experience with Persons Living with HIV/AIDS

  • Service linkage workers must have a minimum of 1-year paid work experience with PLWH.

  • Knowledge of Ryan White grants and services preferred

  • EPIC experience preferred

  • Bilingual / Spanish fluency preferred
