Legacy Community Health is a premium, Federally Qualified Health Center (FQHC) that provides comprehensive care to community members regardless of their ability to pay. Our roots began in 1981 as the Montrose Clinic, with specialization in HIV education, testing, and treatment. Since then, the agency has expanded to 10 clinics in Houston, one in Baytown, two in Beaumont, and one in Deer Park with extensive services that include: Adult primary care, HIV/AIDS care, pediatrics, OB/GYN and maternity, dental, vision and behavioral health. Job Description
The Nutrition Coordinator will be responsible for the day to day operations of the Nutrition Department by assisting the Lead Registered Dietitian with administrative tasks.
Essential Functions
- Order and stock nutrition supplements.
- Dispense supplements with appropriate education and documentation.
- Answer and return patient and provider phone calls.
- Conduct BIA, height and weight.
- Organize and track outside referrals.
- Schedule new and follow up patients.
- Track frequent price changes with supplements.
- Coordinate monthly grant reporting in coordination with accounting team and IT team.
- Conduct data entry and billing for supplement dispensing through Centricity.
- Act as liaison between patients and Registered Dietitian.
- Assist the Registered Dietitian with developing and maintaining policies and procedures for the program.
- Ensure Standards of Care are followed regarding dispensing of supplements.
- Assist with performance improvement activities, including patient surveys.
Education & Training Requirements
- Must have a high school diploma or equivalent.
Work Experience
- Basic computer skills, including MS office and Excel
- Experience working in health care a plus
- Bilingual Spanish/English preferred.
