School Based Provider - Nurse Practitioner/Physician Assistant

Overview

Are you a pioneering healthcare professional passionate about making a difference in the lives of young people? Legacy Community Health is looking for a dynamic School Based Provider to join our ambitious School-Based Program. This role offers the opportunity to administer innovative care to children and support their families within a school-based clinic environment, all the while contributing to a healthier community fabric.

Based in the heart of Houston, Texas, your work will be integral to locations such as KIPP Northeast and YES Prep Fifth Ward. You'll be at the forefront of community healthcare, working Monday through Friday during school hours, allowing you to strongly influence the wellbeing of future generations.

Key Responsibilities

  • Administer forward-thinking care to children within the School-Based program clinic setting.
  • Provide comprehensive healthcare support to family members of enrolled students.
  • Collaborate diligently under the supervision of physicians to deliver superior healthcare outcomes.
  • Work in tandem with the School Based Team to drive health initiatives and educational support across school sites.

Qualifications and Experience

  • Successful completion of a relevant program with an accredited school.
  • Current active license from the Texas Board of Nursing or Texas Physician Assistant Board.
  • Minimum of 2 years paid experience in a healthcare provider role.
  • Bilingual capabilities, particularly in Spanish-speaking, preferred to enhance community engagement.
