As a Care Team Assistant (CTA), you will be the linchpin of communication and support, bridging the gap between patients, care teams, and community resources. This pivotal role ensures the seamless delivery of quality, patient-centered care adhering to the Patient Centered Medical Home (PCMH) model. Your bilingual skills will amplify your impact, enhancing patient engagement and understanding.

Key Responsibilities

  • Patient Communication: Skillfully manage patient interactions via phone or protocol, liaising with nursing staff to provide accurate and compassionate responses to inquiries.
  • Pre-Visit Planning: Conduct thorough reviews of patient schedules ensuring all necessary records are available for proficient preparation.
  • Care Coordination: Assist in guiding patients through their care journey, managing transitions and maintaining the Care Coordination Desktop.
  • Medication Management: Support patients in medication refills and facilitate prior authorization processes.
  • Form Completion: Initiate and prepare medical forms for provider reviews, ensuring accuracy and completeness.
  • Referral Support: Aid the Referral Coordination staff in processing and completing necessary referrals efficiently.
  • Team Collaboration: Foster effective team relations and actively participate in necessary safety and educational programs.

Qualifications

  • High-school diploma or equivalent.
  • Medical Assistant Certification or at least 2 years of clinical experience in an inpatient or outpatient setting.
  • Must maintain BLS/CPR certification.
  • At least 2 years of experience in an ambulatory clinical setting preferred.
  • Proficiency in bilingual communication, catering to diverse linguistic needs for improved patient interaction.
