Health Navigation Specialist

As a Health Navigation Specialist, you will play a pivotal role in bridging gaps between our diverse community and essential healthcare services. In collaboration with our Legacy Pharmacy and healthcare teams, your role will be vital in aligning with our mission to enhance quality of life for everyone, regardless of their ability to pay.

Today, with over 50 clinics throughout Houston and neighboring areas, we offer expansive services ranging from adult primary care to behavioral health, impacting students and families alike. Your role as a Health Navigation Specialist perfectly complements this legacy, driving change and fostering healthier communities.

This position may be partially or fully funded by grants, reflecting our emphasis on innovative and sustainable health solutions.

Core Responsibilities

  • Implement dynamic health education programs in both individual and group settings to empower patients and enhance their health literacy and management skills.
  • Utilize cutting-edge techniques like motivational interviewing and disease self-management to meet diverse health goals.
  • Provide crucial information, referrals, and service linkages to boost patient autonomy and health outcomes.
  • Conduct community and clinic-based point-of-care testing for HIV and other health conditions.
  • Contribute to innovative health education programming development based on community interactions.
  • Act as an integral member of an interdisciplinary care team, cultivating strong relationships with stakeholders.
  • Perform quick assessments of patient requirements, readiness for health behavior change, and track progress.
  • Establish robust referral systems and community networks for effective patient service linkage.
  • Promote patient-centered, culturally sensitive care within a primary-care medical home framework.
  • Document patient interactions meticulously in electronic health records.
  • Manage patient schedules effectively, adhering to performance metrics and grant requirements.
  • Ensure high standards of customer care for an optimal patient experience.

Qualifications

  • High School Diploma or equivalent, with 3-5 years of relevant experience.
  • Bachelor's Degree in a related field with at least one year of relevant experience preferred.
  • Proficient in performing point-of-care testing in clinical or community settings.
  • Demonstrated commitment to HIV care and prevention initiatives.
  • Experience in educating clients from diverse cultural and backgrounds in various settings.
  • Ability to display core competencies in HIV and HIV Prevention within three months of hire.
