Care Coordinator - School Based Health (Bilingual) - Job Overview

Schedule: Monday-Thursday (7AM-4:15PM) and Friday (7AM-3PM)

Key Responsibilities

  • Engage daily with students, parents, and guardians through innovative communication channels including phone, email, and face-to-face interactions.
  • Collaborate with the SBHC team and community organizations to execute a dynamic care plan enhanced by digital health summaries and action plans.
  • Integrate health information technology for proactive care delivery and continuous monitoring.
  • Facilitate seamless navigation across health, educational, and community systems for students and families.
  • Leverage data collection and analytics to connect students and families to community support resources efficiently.
  • Harness cultural and language assets to effectively address student and family needs.
  • Maintain compliance through meticulous care coordination documentation.
  • Innovate outreach strategies at school events to enhance service utilization.

Minimum Qualifications

  • High School Diploma or equivalent is mandatory; post-hire completion of specialized training programs required.
  • Minimum of two years of experience within health care, schools, or community systems.
  • Associate's or Bachelor's degree preferred; experience in care navigation or direct patient care is advantageous.
  • Strong organizational and communication skills with a focus on empathy and cultural humility.
  • Proficiency in digital health platforms and data documentation.
  • Experience with diverse populations and a collaborative team mindset.
  • Bilingual proficiency in English and Spanish is highly valued.
