About the Position

The School-Based Health Care Coordinator connects students and their families to health and social resources within the school, school-based health center, and community. Supports an interdisciplinary team to develop tailored plans that support the student’s academic performance and wellness, enhances communication and relationships across the school community, and fosters improved health outcomes and educational success for students.

Requirements

  • High School Diploma or equivalent is required.
  • After hire, must complete School-Based Health Alliance, Legacy Community Health, and school standardized training. 
  • Previous work experience navigating health care, schools, and community systems or programs is required.
