Legacy Community Health is hiring immediately for Patient Information Coordinators through our partnership with the AmeriCorps Program. AmeriCorps is a Federal Program that connects Americans with Community Services in Health, Public Safety, etc.
The session begins September 3, 2024. Must commit to completing the 1700-hour requirement by August 31, 2025. The bi-weekly (every two weeks) stipend is $700, less applicable taxes.

About the Position
Helps reduce barriers to healthcare by helping patients navigate primary healthcare services, including assistance in coordinating healthcare visits.
Key Responsibilities
- Identifies patients who may experience barriers to accessing healthcare, particularly for those unfamiliar with Legacy systems.
- Greets and assists patients, their families, and visitors entering the facility with a positive and helpful attitude.
- Answers questions, provides information, and directs people to the area needed, thus creating an atmosphere in the facility that is supportive and efficient.
- Assists patients by providing wayfinding information for patients by accessing job resources.
- Assists the patients/visitors to the assigned patient care area.
- Performs other duties as assigned.
- Represent Legacy’s AmeriCorps Program by wearing the AmeriCorps logo daily
- In the event of a local, state, or federal disaster declaration, AmeriCorps members may be asked on a volunteer basis or be required by OneStar to activate to assist in disaster response and recovery activities within their communities. This may involve virtual service opportunities or in-person service opportunities. This may include service opportunities outside of the member’s standard service hours or in place of their standard service duties. In the event of a large-scale, high needs local, state, or federal disaster, AmeriCorps members may voluntarily or be required by OneStar to deploy to an area outside of their service area to assist in disaster response and recovery activities.
- Work with community partners to identify service opportunities in alignment with the program design.
- Members may be asked to perform other position-related instructions within the scope of their normal service activities as requested by their site supervisor and/or AmeriCorps program staff in alignment with the grant and not unallowable or prohibited activities and subject to reasonable accommodation.
- Provide translation and interpretation services as appropriate.

Since then, the agency has expanded to >50 clinics in Houston, one in Baytown, two in Beaumont, and one in Deer Park with extensive services that include: Adult primary care, HIV/AIDS care, pediatrics, OB/GYN and maternity, dental, vision and behavioral health. Requirements
- High School diploma or GED is required.
- No previous experience is required.
- Bilingual / Spanish fluency is preferred.
- U.S. Citizen or a Permanent Resident
