Legacy Community Health is hiring immediately for Community Health Workers through our partnership with the AmeriCorps Program. AmeriCorps is a Federal Program that connects Americans with Community Services in Health, Public Safety, etc.

The session begins September 3, 2024. Must commit to completing the 1700-hour requirement by August 31, 2025. The bi-weekly (every two weeks) stipend is $700, less applicable taxes.

About the Position
Responsible for outreach, education, navigation, and preventive health screenings in neighborhoods located in the Southwest area of Houston, Texas.

Key Responsibilities

  • Assist in the assessment of community health needs through utilization of nationally recognized social determinants of health screening tools.
  • Establish rapport with new refugee and immigrant community members and families.
  • Conduct home/site visits to identify needs for services and provide information and referrals to diverse resources.
  • Maintain case record documents providing thorough documentation in a timely manner.
  • Promote Legacy service lines and link community members to Legacy as a medical home.
  • Serve as peer trainer for new or less experienced members.
  • Maintain partnerships with Connect Community.
  • Perform other duties as assigned.
In addition to the activities above, all Legacy Community Health Corps Members will:
  • Represent Legacy’s AmeriCorps Program by wearing the AmeriCorps logo daily
  • In the event of a local, state, or federal disaster declaration, AmeriCorps members may be asked on a volunteer basis or be required by OneStar to activate to assist in disaster response and recovery activities within their communities. This may involve virtual service opportunities or in-person service opportunities. This may include service opportunities outside of the member’s standard service hours or in place of their standard service duties. In the event of a large-scale, high needs local, state, or federal disaster, AmeriCorps members may voluntarily or be required by OneStar to deploy to an area outside of their service area to assist in disaster response and recovery activities.
Nonessential Functions: Legacy Community Health Corps Members may have opportunities to engage in the following activities:
  • Work with community partners to identify service opportunities in alignment with the program design.
  • Members may be asked to perform other position-related instructions within the scope of their normal service activities as requested by their site supervisor and/or AmeriCorps program staff in alignment with the grant and not unallowable or prohibited activities and subject to reasonable accommodation.
  • Provide translation and interpretation services as appropriate

Requirements

  • High School diploma or GED is required.
  • No previous experience is required.
  • Bilingual / Spanish fluency is preferred.
  • U.S. Citizen or a Permanent Resident
